Although every client is automatically evaluated when we create their customized treatment plan, HEART also offers evaluations that may be required by a third party (work, school, insurance, etc...).  Here are all the types of evaluations we offer.

Driver's License Evaluations

Adult SUD Evaluations

Substance use evaluations are a vital step in understanding your needs and determining the correct level of care.  Whether it’s individual therapy, an Intensive Outpatient Program (IOP), or a Partial Hospitalization Program (PHP), or a group, we’ll help you take meaningful steps toward recovery.  HEART has eight different programs to suit your needs.
